Grant Application

We have created three types of files for your application. Please download one of these files, fill it out, save the file and email as an attachment to Choose either the Excel for Mac format, the Excel for Windows format or the PDF format.

The form is restricted to comply with the original format of the application. Information that does not fit within the space given on the form can be submitted via a one-page supplemental document. A cover letter may also be included in your sumbission. The Foundation prefers applications to be emailed to if possible, but also accepts applications via US Mail that arrive prior to the deadline. Applications must be received by February 1, May 1, August 1, or November 1 to be considered at the ensuing quarterly meeting. If applications are not received by the indicated dates, the application will be added to the next quarter.

The tax exempt number requested is that provided by the IRS (not the state of Wisconsin).  In addition to the number, provide a copy of the IRS letter with the application. Incomplete applications will not be accepted.

School requests must be channeled through the superintendent, who will establish priorities and submit the applications.

The Board of Directors will determine the grants, based upon available funds, guidelines, and the impact of the grant on the community.

Notification of acceptance or rejection of requests will be made by phone, e-mail, or US Mail after the board meeting.
